The position
As a social psychiatric nurse / community nurse with Team
Vangnet, you’ll work in the community with Amsterdam residents who need extra support.
You’ll visit clients at home, respond to reports from housing authorities,
the police, or neighbors, and assess what care is appropriate. Sometimes this involves
complex issues or acute situations, in which case you’ll provide immediate care or
call in specialist help.
You will be called upon in situations where a social psychiatric nurse
(SPN) is needed, such as persistent disturbances or complex issues. In these
crisis situations, coordination with colleagues and partners in the care chain is essential. You
will do this by, among other things:
- to follow up on reports in consultation with, for example, the community police officer,
the housing authority, or local residents, and to assess the situation on site;
- to provide acute care in response to requests from the police or social
service organizations and, if necessary, to refer clients to
specialized or additional care;
- participate in multidisciplinary consultations with partners in the care continuum
and coordinate care.
You will work independently with your own caseload and regularly coordinate with
colleagues and partners in the care continuum. Depending on your education and experience, you will either start
immediately as a Psychiatric Support Specialist (SPV), or begin as an outpatient nurse with a
training program designed to help you advance to the role of SPV or Master of Mental Health Nursing.
Here's what you do on an average day
- Active participation in multidisciplinary meetings to coordinate and assign
the day’s tasks.
- Manage your own schedule, visiting clients both on-site
and at their homes.
- Assessing clients’ care needs and helping to
organize their care.
- Establishing and monitoring care coordination, with a focus on the client’s
overall situation.
- Providing consultations to, among others, partners in the care network, neighbors, and
agencies.
- After your training period, you will start working regular shifts.
